Waiver of Liability and Hold Harmless Agreement
The Safety Committee is asking the congregation, and the employees, to sign the Waiver of Liability and Hold Harmless Agreement. A copy of this Waiver can also be downloaded, see file below. This waiver applies to everyone, including employees, and must be returned to the office for filing. We need one waiver submitted for each adult, age 16 or older. Children under the age of 16 may be added to one of the parent’s waiver. You may bring the completed waivers with you on Sunday; mail a copy; or drop it off at the church, M-F between 8:00 am – 3:00 pm – please call first.
If you plan on using Fellowship Hall for a private gathering, you must reserve the space by calling or e-mailing the office. All the guests attending the function must sign a waiver. The office will provide blank forms to the Host, who will be responsible for getting the signatures and returning the signed documents to the office.
